ABSTRACT
Objective To explore the effectiveness of modified Bardach two-flap palatoplasty combined modified Furlow double-opposing Z-plasty in wide palatal cleft repair.Methods 80 patients aged 2-18 years old with Ⅲ cleft palate from our department were divided into two groups,the experimental group (n=40) was repaired with modified Bardach two-flaps palatoplasty combined modified Furlow double-opposing Z-plasty (modified B+F technique).The control group (n =40) was repaired with Sommerlad levator muscle of palatine velum reconstruction (S technique).Results The healing of incisions was better in the experimental group than the control group.The fistula rate in the experimental group was 2.5％,which seemed to be remarkable lower than that of the control group (7.5％) post operation one month.Conclusions Application modified Bardach two-flaps palatoplasty combined modified Furlow double-opposing Z-plasty in wide palatal cleft will promote healing of incisions,reduce the occurrence rate of cleft palate fistuia,inhibit the scar contracture of soft palate,maintain the length of soft palate,augment movement of soft palate,and improve the speech quality of patients.

Objective To analyze the MRI features of hydrosalpinx and to investigate its clinical value.Methods MRI and ultrasound manifestations in 40 patients with hydrosalpinx in 53 fallopian tubes confirmed by operative and pathological findings were analyzed retrospectively,and these findings were also compared with the results of aparoscopy and pathology.Results Among 53 fallopian tubes with hydrosalpinx,bilateral tubes in 13 patients and 27 unilateral tubes in other patients were found.The tubes were botuliform in 32, retort-shaped in 1 6,pouch-shaped in 5.Incomplete separation of the lumen were found in 1 9.In 9 patients with acute salpingitis,1 5 tubes were found with empyema and expansion.In other 31 patients with chronic salpingitis,38 tubes were with hydrops and expansion,14 of whom were with hematocele.The sensibilities of MRI and ultrasound diagnosis for hydrosalpinx were 94.3%(50/53)and 88.7%(47/53)with no statistical differences(P >0.05),however the specificitis were 90.6%(48/53)and 77.6%(41/53)with obvious statistical differences(P <0.05).Conclusion The locating and qualitative diagnosis of MRI for hydrosalpinx is superior to ultrasound.MRI can discriminate the quality of cyst fluid and thus can definitely diagnose the hydrosalpinx caused by acute or chronic salpingitis.

Objective:To determine the level of FIB,IL-6,D-dimer and explore the differential diagnostic value and clinical significance of the tumor markers in patients with ovarian cancers.Methods: The level in serum of 65 ovarian cancer and 35 benign tumor of ovarian and 30 controls FIB and D-dimer were measured by latex agglutination assay.The IL-6 levels were measured by ELISA method.Results:Compared with the normal controls and benign tumor of ovarian cancer, there were some significantly changes in ovarian cancer patients including FIB,IL-6,D-dimer increased (P0.05).Companied with TNM stage increased,the levels of FIB,IL-6,D-dimer ovarian cancer groups increased.The correlation analysis showed that FIB was positively correlated with D-dimer and IL-6 (r=0.56,r=0.31).Conclusion:The level of serum FIB,D-dimer,IL-6 detection in patients with ovarian cancer are helpful in early diagnosis, dicision of clinical stages and regimen prognosis.

Objective To discuss the preventive measures of anterior palatal fistula by modified the operation of cleft palate. Methods For 23 patients of complete cleft palate with alveolar ridge cleft, bilateral mucoperiosteum flap was moved forward as possible so as to close fistula front of hard palate, and incised junction of the hard and soft palate to prolong soft palate and the wound was repaired by buccal mucosal flap.The incidence of anterior palatal fistula and velopharyngeal closure after operation was observed. Results Twenty-three patients were rechecked 1 month after operation ,there was no anterior palatal fistula occurring,10 cases were examined by epipharyngoscope 1 year after operation,the velopharyngeal closure was 90%-100%. Six cases were followed up for 6 months,the velopharyngeal closure was 80%-85%. Conclusions For second-stage operation methods of anterior palatal fistula, there are too many discussions of selection criterias, advantages and disadvantages. If first-stage operation is taken measures to prevent anterior palatal fistula or decrease the diameter of fistula as possible. It reduces percentage of second-stage operation or decreases the difficulty. It should get more attention in the clinical works.

ObjectiveTo analyze the effect of lowering the upper limit of normal fasting glucose on the distribution of impaired gheose regulation(IGR)in the population in Wuhan urban area.MethodsThe health profiles of 1896 people after physical examinations were analyzed by X2 test.ResultsThe low limit for impaired fasting glucose(IFG)was decreased from 6.1 to 5.6 mmol/L,IFG increased 271 patients.the prevalence of IFG increased from 13.1%to 27.4%(X2=62.68,P<0.05),and the value of normal glucose test decreased from 60.3%to 47.5%(X2=120.03,P<0.05).Overweisht or obesity,hypetention,fatty liver,hyperlipidemia,ECG abnormality were significantly different(P<0.05)in groups of fasting glucose 5.6-6.0 mmol/L to<5.6 mmol/L and to 6.1-6.9 mmol/L ConclusionThe new cut point of IFG has significantly influenced the distribution of IGR in the population of Wuhan urban areas.In the newly IFG.increased subjects,metabolic abnormalities of glucose and lipid occur,and an intervention of life-style is needed for them.

To investigate the seropharmacological action of Buyang Huanwu Decoction (BHD) on hypoxia induced apoptosis of cultured cortical neurons. Models of hypoxia induced neuron apoptosis were established by Daniel method. Neuron apoptotic rate was examined by propidium iodide staining method and flow cytometry, nitric oxide (NO) and malonyl dialdehyde (MDA) contents by spectrophotometer and bcl 2 gene expression by immunohistochemical method. Effects of BHD on neuron apoptotic rate, NO and MDA contents and bcl 2 gene expression were observed in normal group (Group A), model group (Group B), normal saline group (Group C) and high dosage, moderate dosage and low dosage BHD groups (Group D, Group E and Group F). BHD could inhibit the apoptosis of cultured cortical neurons, reduce the production of NO and MDA and promote the expression of bcl 2 gene.[Conclusion]BHD has an inhibitory effect on hypoxia induced apoptosis of cultured cortical neurons and its mechanism may be concerned with the reduction of NO and MDA production and the increase of bcl 2 gene expression.
